Your journey with CANARC begins by completing our patient inquiry form. Once we receive your information, a member of our Patient Care Team will contact you to answer your initial questions and guide you through the next steps of the consultation process.

You will be assigned a dedicated Patient Care Coordinator who will serve as your primary point of contact, taking the time to understand your diagnosis, medical history, treatment experience, and individual care goals. To support a comprehensive clinical assessment, you will be asked to submit your relevant medical records, including pathology reports, laboratory results, imaging studies, and treatment history.

Our multidisciplinary clinical team will carefully review your medical information and develop a personalized integrative care plan tailored to your specific needs. Once your assessment is complete, your Patient Care Coordinator will schedule a consultation with one of our healthcare professionals to discuss your recommended care plan, answer your medical and non medical questions, and explain the available services and support options.

After you have reviewed your personalized care recommendations and decided to proceed, your Patient Care Coordinator will assist you with scheduling your appointments, completing the required documentation, and coordinating every aspect of your consultation experience with CANARC, ensuring a smooth and well supported journey from your first contact onward.

To provide the most appropriate recommendations, we request your most recent medical records before your consultation. These records enable our multidisciplinary clinical team to assess your current health status and develop a personalized integrative care plan tailored to your clinical needs.

Where available, please submit recent laboratory results, pathology and biopsy reports, diagnostic imaging reports such as CT, MRI, or PET scans, and your treatment history.
